USAGE: This field is conditional.

NOTE 1: Required when the first position of the TQ field on the ASR Access Service Request Form is "C".

NOTE 2: Optional when the first position of the TQ field on the ASR Form is "M" or "X".

NOTE 3: Otherwise prohibited.
