INIT - Initiator

Identifies the initiator as specified on the ASR Access Service Request Form by the customer.

NOTE 1: The INIT field entry must be identical to the INIT field entry on the ASR Form.

USAGE: This field is conditional.

NOTE 1: Required when the INIT field on the ASR Form is populated, otherwise prohibited.

DATA CHARACTERISTICS: 15 alpha/numeric characters
